Change of address or replacement personal licence
To change your address or request a replacement personal licence you will need to provide (upload):
- one passport-sized photograph that needs to be 'countersigned' by somebody else to prove the identity of the person applying – a countersignatory must meet the following criteria. The endorser must then sign, date and print their name, and state their professional qualification on the back of one of the photos and the endorsement should be written ‘I certify that this is a true likeness of [the full name of the person]'. The countersignatory also cannot be related to you, be in a relationship with you or live at your address
- an upload of your signature with the date
You must destroy your previous licence. The fee for the amendment is £10.50.

Change of name
If your amendment is a change of name you must provide (upload):
- supporting documents – these can be a marriage certificate, a deed poll certificate or a photograph ID with your new name
- one passport-sized photograph that needs to be 'countersigned' by somebody else to prove the identity of the person applying – a countersignatory must meet the following criteria. The endorser must then sign, date and print their name, and state their professional qualification on the back of one of the photos and the endorsement should be written ‘I certify that this is a true likeness of [the full name of the person]'.
